A fibroid (leiomyoma) is a common benign growth / tumour of muscle in the uterus (womb). These growths may occur on the outside (subserosal fibroid), in the body (intramural) or in the cavity (submucosal) of the uterus.

1 in 6 women have fibroids. Not all fibroids cause gynaecology or fertility problems and not all fibroids need medications or surgery.

Some fibroids have an effect on the ability to fall pregnant. The effect on fertility is likely to be related to the fibroid size and position of the fibroids.

Fibroids can grow in size during pregnancy and may cause pain due to fibroid degeneration.

Fibroid position appears to influence fertility, therefore investigation must be able to exclude uterine cavity involvement by the fibroid. 3D pelvic scan can be very useful in diagnosis than the 2D normal transvaginal ultrasound (Scan). MRI or hysteroscopy are preferred.

Effects of fibroids on fertility and pregnancy outcomes:

Subserous fibroids do not appear to have a significant effect on fertility outcomes.

Intramural fibroids may be associated with reduced fertility and an increased miscarriage rate.

SM fibroids are associated with reduced fertility and an increased miscarriage rate. Hysteroscopic myomectomy for SM fibroids is likely to improve fertility outcomes.

Indications for myomectomy in infertile women

  • Infertile women and those women undergoing fertility treatment and have Sub mucous fibroid(s) should consider removal of fibroids of any size.
  • Infertile women with symptomatic fibroid(s), such as heavy vaginal bleeding or pressure symptoms can consider removal of fibroids.
  • Couples presenting with failure to conceive and / or had miscarriage with intramural fibroids size >4 cms or multiple failed cycles of fertility treatment can consider removal of fibroids.

Many women do conceive naturally after the fibroid removal surgery.

Uterine artery embolisation (UAE),and other mode of treatments are not indicated if women wants to conceive.

Myomectomy is the accepted surgical management for fibroids that are felt to influence fertility. This may be via a laparotomy, laparoscopic or hysteroscopic approach depending on the size and location of the fibroid.

Role of medical management of fibroids in women desiring future fertility

Medical management of fibroids delays efforts to conceive and is not recommended for the management of infertility associated with fibroids.

However, short term use of a GnRH analogue can be useful for pre-operative correction of anaemia or short term reduction in fibroid volume.

The use of ulipristal acetate in infertile women with fibroids is not recommended due to the risk of significant adverse side effects.

We visited Dr. Basavaraj for our treatment. We had to visit for over an year and couple of procedures. Doctor is very knowledgeable and has vast experience. Nurses and other lab staff are good. Initially the wait time was less ,however off kate the wait time is on higher side since doctor does scan and procedures in parallel. The wait time doesn't change even with prior appointment . This is especially true in Marathahalli centre. This can be improved. I feel cost here for treatment is much lesser than any corporate hospitals or other centres. Doctor is mostly straight forward and not verbose. Also doctor prescribes just enough medication which might or might not be good, hard to judge.

Dr Devarshetty has a midas touch in his treatment apart from his knowledge skills as a infertility specialist. I recommend all patients to be open minded while consulting him. My wife had a very difficult phase in conceiving after getting ectopic pregnancy twice in 2015-16 because of her PCOD . Being a Dr myself i couldn't cope up with the stress with which i and my wife underwent during those times, later in 2016-17 after Dr Devarshetty advise and treatment we finally became parents and it was the most joyous moment of our married life. Now we have a healthy baby girl 10m old..posted mother and baby image.
